The honest answer requires separating two very different systems: NHS prescribing and private clinic prescribing, which operate by entirely different rules.

The Three NHS Conditions

  1. Treatment-resistant epilepsy in children — Dravet syndrome and Lennox-Gastaut syndrome (Epidyolex, CBD-based).
  2. Chemotherapy-induced nausea where conventional antiemetics have failed (Nabilone, synthetic THC).
  3. MS spasticity — Sativex (sublingual THC/CBD spray).

Even for these three, NHS prescribing is vanishingly rare.

Private Clinic Indications

Chronic Pain (approx. 70% of private patients)

Neuropathic pain, musculoskeletal pain, fibromyalgia, arthritis and chronic migraine. Most clinics require documented evidence of prior treatment failure.

PTSD

Evidence from Israeli military studies and US research from veteran populations drives growing acceptance. UK private clinics increasingly accept PTSD as a qualifying condition.

Anxiety Disorders

CBD-dominant or balanced THC/CBD preparations. Clinics vary in how readily they accept anxiety as a standalone indication.

ADHD

Adult ADHD is an emerging indication for patients who cannot tolerate stimulants.
